I'm 5' 6" and 166. Max. weight standards for my height is 146. I want to get to about 140. Before I start my personal workout or change my diet, should I talk to my doctor first, to prevent anything going wrong in my dieting and exercise program?

I don't think there's a doctor in the world that wouldn't support eating a healthier diet and increasing your physical activity. If you limit calorie intake and eat more fruits and vegatables, drink more water instead of anything else, and run a few times a week, you'll lose the weight. If you're considering anything more drastic than that, it's never a bad idea to talk to your doctor. I'm sure that your doctor will be pleased that you want to improve your physical fitness level and thereby improve your health.
